Along with the supporting documents as listed in the applications, please also attach the aircraft or flight simulator rental document, as well as the Proof of Services or Service Invoice or any other document for the flight examination. For cases where the examination is carried out by Flight Examiners who are employees of commercial airlines and examine pilots of the company they work for, as well as Examiners, employees of ATOs for the examination of pilots and registered trainee pilots of the particular ATO, a supporting document will be provided by the school or company which will confirm the above statement. All proof of payment receipts and/or invoices shall be forwarded to the relevant oversight authorities of the State, if deemed necessary.
